Thank you for your interest in ASA College Miami Athletics!

The initial step in your registration process begins with completing our recruit questionnaire. YOU MUST COMPLETE A RECRUIT QUESTIONNAIRE.

1. Scan & email the following documents (except official transcripts, they must be sent by high school/college, e-transcripts are accepted)

  • Copy of your Valid State ID, Passport or Alien Registration Card
  • Copy of your Social Security Card
  • Copy of your Birth Certificate
  • High School or GED diploma & sealed official high school transcript (HS Grads) - send to snatal@asa.edu
  • Sealed official college transcript (College Transfers)
  • Immunization Records (MMR)
  • Signed copies of parent's tax forms from previous year (W2 forms if married)
  • Copy of parent's valid state ID or Passport
  • $25 Application Fee (Cash/Check/Money Order)

  • Your Email
